: To update the address, demographic, or emergency contact information we have on file for your student(s), please complete this form in its entirety. Once the form is submitted, you will also need to submit updated proof of residence documentation for all address change requests or legal documentation from the Court for all legal name changes to the YPS Enrollment Center as instructed on this form.

EN-TRA001 YPS OKSDE Student Transfer Form
: If your student transfer request has been denied, you may request that the YPS Board of Education review the decision to ensure the process was followed correctly at their next regularly scheduled board meeting. In order to request reconsideration of a denied transfer request you must complete this form within ten working days of receiving your denial letter.
