Oklahoma Preventions Needs Assessment Survey

6th -   8th Grade:  May 11, 2012
9th - 12th Grade:  May 14, 2012

Yukon Middle School and High School students will be taking part in the Oklahoma Preventions Needs Assessment Survey May 11, 2012, and May 14, 2012.  The purpose of the survey is to gather information needed to plan prevention and intervention programs in the Yukon community and Yukon schools. These programs will address problems of alcohol, tobacco, and other drug use, school dropout, delinquency, and violence in our schools and communities.  The survey is completely voluntary.  Students may refuse to take the survey and parents may refuse to have their child take the survey.  Letters will be given to each student to take home to their parents/guardians with detailed information.  If you have questions, you may contact John Parker, the Project Coordinator, at 350-2630, Ext. 1860.
